It is humbling to reflect that it is more than 20 years
since the First Edition of this textbook. Our mission then,
and our mission now, was to produce a readable, yet
scholarly textbook that can serve as a primary resource
for clinicians addressing the intestinal, hepatic, and
nutritional health of infants, children, and adolescents.
It is important for all clinicians to remember that the
diagnosing of gastrointestinal and liver disease starts with
a careful history and physical examination, and being
able to retrieve detailed descriptions of these elements in
a single location will facilitate care. For novices to the
field as well as for experienced clinicians, it is invaluable
to have a primary resource even in this age of instant
electronic searches. As powerful as today’s search engines
are, they cannot substitute for a well-written comprehensive
textbook.
Knowledge has moved forward on many fronts over
the past two decades. In the early 1990s, clinical trials on
biologic therapy for inflammatory bowel disease were
barely beginning. The characterization and importance
of the intestinal microbiome was in its infancy. Today,
however, the increasing array of monoclonal antibody
therapies for Crohn’s disease and ulcerative colitis holds
promise of changing natural history, and manipulation
of the intestinal microbiome may be useful for treating
diseases such as obesity, diabetes, and inflammatory
bowel disease.
A continuing strength of this book has been the panel
of superb authors who have selflessly given their time and
efforts to contribute chapters that incorporate important
updates to their topics while not losing focus on diagnosis
and treatment. The book continues to be organized
into distinct sections starting with basic aspects of gastrointestinal
function, followed by common clinical problems,
organ-specific diseases, surgical procedures, and
gastrointestinal procedures. The last three sections of the
book focus on liver disease, pancreatic disease, and nutritional
issues.
We have also greatly expanded our Board Question
resource within the electronic version of this textbook. It
is our hope that these questions, along with our chapters,
will aid in review for board examinations for clinicians
at all levels.
We have been fortunate to work with a talented group
of individuals from Elsevier over the years. Special thanks
go to Suzanne Toppy, Lisa Barnes, and Ted Rodgers for
helping to make this an outstanding edition.
